29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:    East Dock, Overhead Door No. 6 - On or about November 17, 2016, a maintenance employee was required to install a hydraulic hoist cylinder that lifts and lowers the automatic dock platform. The employer's energy control procedures utilized by the employees for the orderly shutdown of the equipment (dock leveler) and isolation of hazardous energy did not clearly and specifically outline the scope, purpose, authorization, rules and techniques including, but not limited to, the following:    a. Identify the types and magnitude of energy (hydraulics and kinetic).  b. Identify the types and locations of energy isolating devices (maintenance locks / props and storage lock / prop) and the lockout hardware (size and grade of the bolts and nuts).   c. Torque 50 to 100 foot pounds the hardware (bolts and nuts) used to lockout the maintenance locks (props) in the service position (vertical).  d. Identify the methods to dissipate and restrain the hydraulics and the kinetic energy of the dock platform.  e. Identify the means to enforce compliance of the employer's lockout / tagout work rules.    Thereby employees were exposed to struck-by hazards.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:    A) Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of energy available in the workplace, and the methods, devices, and means necessary for energy isolation and control.    a) East Dock, Overhead Door No. 6 - On or about November 17, 2015, a maintenance employee was required to secure a dock platform while it was in the service position (vertical) in order to perform repair work (install a hydraulic hoist cylinder that lifts and lowers the dock platform). Employee training specific for this task did not include the following:    1.) Turn off and isolate the ball valves that supply hydraulic fluid to the hoist cylinder that lifts and lowers the platform.    2.) Physically identify the size and grade of the lockout hardware (bolts and nuts) required by the manufacturer.     3.) Ensure that the employees understand that the lockout hardware (bolts and nuts) used to secure the maintenance locks (props) shall not be used for other purposes.     4.) Torque 50 to 100 foot pounds the hardware (bolts and nuts) used to lock out the maintenance locks (props) in the service position (vertical).    b) Throughout facility - On or about November 17, 2015, employees (janitors) were required to remove debris and broom sweep the concrete pit beneath the dock platform. The employees (janitors) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of energy available in the workplace, and methods, devices, and means necessary for energy isolation and control.    Thereby employees were exposed to struck-by hazards.
