29 CFR 1910.179(n)(3)(i): The load shall be well secured and properly balanced in the sling or lifting device before it is lifted more than a few inches: On or about 11/02/2015, an employee from SAF Holland Inc. located at 404 Booneslick Road, Warrenton, MO, was unloading metal coils from a trailer. Due to the close placement of the coils on the trailer, the employee(s) had to utilize a board (approx. 4 inch X 4 inch) to separate the coils during the unloading process. The board was placed by hand on the C shaped coil lifting device. During this process a caught-by pinch point was created which amputated part of an employees finger. This C shaped lifting device was manufactured as a stand-alone lifting device. The load should be secured and balanced by the lifting device itself.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA: On 11/02/2015, an amputation incident occurred at SAF Holland. The employer reported the incident on 11/13/2015. The employer did not report the amputation incident within twenty-four (24) hours of the hospitalization.
