29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) On or about 11/16/15, at the roof area of the site, Lancaster, NY. Employees, engaged in roofing work on a 5 in 12 roof, were not protected by fall protection system(s). Employees were exposed to fall of 11.5 feet.   NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) On or about 11/16/15, at the site of Lancaster, NY. The ladder, used to access the roof, did not extend the side rails 3 feet above the upper landing. Employees were exposed to fall of 11.5 feet.   NO ABATEMENT CERTIFICATION REQUIRED   The employer, Sahlem's roofing & Siding, was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1088453, citation number 1, item number 3-a, and was affirmed as a final order on 10/27/2015, with respect to a workplace located at 511 union Road, West Seneca, NY.
