29 CFR 1926.350(a)(6): Compressed gas cylinders were being moved without being secured on a special carrier intended for this purpose and did not have the regulators removed and valve protection caps put in place: Employees were exposed to struck by and burn hazards when an unused compressed gas cylinder was being transported in the back of a service truck with the regulator still in place.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Employees of Irwin Steel Erectors, Inc. were engaged in roofing work without fall protection and exposed to fall hazards of more than six feet while moving and holding materials on the roof of the structure The employer, Irwin Steel Erectors, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1):, which was contained in OSHA inspection number 1045012, citation number 1, item number 3 and was affirmed as a final order on July 28, 2015 with respect to a workplace located at 441 98th ST Lubbock TX 79412.
