29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. On or about January 5, 2024 employees utilizing a Skyjack electric scissor lift to weld long span joist were exposed to fall hazards in excess of 10ft with out any type of fall protection or safety measure in place.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. On or about December 21, 2023, the employer failed to report an in-patient hospitalization due to employee amputation to the Occupational Safety and Health Administration, U.S. Department of Labor within twenty-four (24) hours.
