The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On or about November 10, 2015 employee who perform servicing and maintenance on or around Pellet Mill conveyer #2 was not required to: (1) Physically locate and operate the electrical disconnect in such a manner as to isolate the machine or equipment from the energy source. (2) Affix lockout or tagout devices to the electrical disconnect.

29 CFR 1910.219(f)(4): Openings for oiling. When frequent oiling must be done, openings with hinged or sliding self-closing covers shall be provided. All points not readily accessible shall have oil feed tubes if lubricant is to be added while machinery is in motion. a) On or about November 10 ,2015 while preforming maintenance on the conveyer system, openings were not attached to the locations for the maintenance of equipment.
