Lima, Ohio: On or about October 28, 2016 the employer failed to ensure that machine specific energy control procedures were developed, documented, and utilized to control the electrical, hydraulic, and compressed air energy sources when employees entered the SMETCO 300 Pallet Sorter to remove or restack fallen wood pallets. Employees were exposed to caught in and crushing injuries in that the electrical, hydraulic, and compressed air energy sources were not isolated.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6 a) Buckeye Diamond Logistics, Inc. Lima, Ohio: On or about October 28, 2016 the employer failed to ensure that employees entering the SMETCO 300 Pallet Sorter to perform activities such as, but not limited to, removing and restacking fallen pallets implemented energy control application steps to isolate the equipment from the hazardous energy sources (electrical, hydraulic, & compressed air). Employees were exposed to caught in and crushing injuries in that the electrical, hydraulic, and compressed air energy sources were not isolated.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) Buckeye Diamond Logistics, Inc. Lima, Ohio: On or about October 28, 2016 the employer failed to ensure that employees who operated the SMETCO 300 Pallet Sorter were adequately trained in the skills required for the safe application, usage, and removal of energy controls. Employees that entered the SMETCO to remove or restack fallen pallets were not trained to isolate energy sources to protect against the expectant energization and/or release of stored energy. Employees were exposed to caught in and crushing injuries in that the electrical, hydraulic, and compressed air energy sources were not isolated.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours: a) Buckeye Diamond Logistics, Inc. Lima, Ohio; On or about October 28, 2016, the employer failed to report to the local OSHA office of an employee hospitalization within twenty-four hours. An employee was hospitalized on October 28, 2016 after their right leg was caught between the cross bar of a conveyor and the walls of an automated straightener. The OSHA area office was notified of the hospitalization on November 08, 2016.
