29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about 11/12/2015 - Production Department of Plant #1, Rovetta Press, Model S4-2000, employee(s) were exposed to crushed-by hazards when all of the steps in the Rovetta Set Up Procedure were not followed including ensuring the area was clear and that "ALL CLEAR" had been called by all affected associates prior to movement of the bolster.
