29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:    Hotel undergoing renovation, west: On or about November 18, 2015, and at times prior thereto, employees without fall protection were exposed to a fall hazard greater than 6 feet, while removing asphalt shingles from the roof without being trained on fall hazard recognition.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:        Hotel undergoing renovation, west: On or about November 18, 2015, and at times prior thereto, employees without fall protection were exposed to a fall hazard greater than 6 feet, while removing asphalt shingles from the roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   Hotel undergoing renovation, west: On or about November 18, 2015, and at times prior thereto, employees were exposed to a fall of approximately 10 feet, while using a 26 foot extension ladder from the first floor roof to access the building roof that had the side rails approximately 1.5 feet beyond the landing, to remove asphalt shingles from the roof.
