29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:       a) On or about 11/19/15, at 11160 SW 225th Terrace, in the city of Miami, an employee at the peak of the roof cutting plywood with a circular saw during sheathing was exposed to a struck-by hazard due to lack of eye protection.    b) On or about 11/19/15, at 11160 SW 225th Terrace, in the city of Miami, an employee near the peak of the roof hammering nails into the plywood while sheathing was exposed to a struck-by hazard due to lack of eye protection.    c) On or about 11/19/15, at 11160 SW 225th Terrace, in the city of Miami, an employee at the edge of the roof cutting lumber for the fascia with a circular saw was exposed to a struck-by hazard due to lack of eye protection.    d) On or about 11/19/15, at 11160 SW 225th Terrace, in the city of Miami, an employee on a roof overhang hammering nails into the lumber was exposed to a struck-by hazard due to lack of eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  On or about 11/19/15, at 11160 SW 225th Terrace, in the city of Miami, employees installing sheathing on a residential roof were exposed to a 19.2 feet fall hazard due to lack of a fall protection system.     b)  On or about 11/19/15, at 11160 SW 225th Terrace, in the city of Miami, employees installing sheathing on a residential roof overhang were exposed to a 10.6 feet fall hazard due to lack of a fall protection system.
