29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) At the job site, on or about November 18, 2015, an employee exposed to overhead hazards was not wearing a hard hat, exposing employee to struck-by hazards.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: a) North side of the building, on or about November 18, 2015, two employees were elevated and working from an unapproved work platform that was supported by a GEHLTelescopic forklift, Model #518947. The forklift manufacturer does not allow this forklift to be used to elevate personnel or be fitted with any form of personnel platform, which exposed employees to fall hazards up to 16 feet.
