29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Worksite located at Sandstone and 214th Streets, Gretna NE; a residential structure:  Employees were observed working on residential structure performing framing activities without any type of fall protection system in use.  This exposed the employees to falls in excess of 10 feet.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     Jobsite located at Sandstone and 214th Streets and Throughout the Company:  The employer failed to train employees that were potentially exposed to fall hazards to enable each employee to recognize the hazards associated with the type of work being conducted.  This should include training on mitigating those hazards through an acceptable method of fall protection.  Training should also include the requirements for fall protection systems and when it is appropriate to utilize those systems.         Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.1053(a)(8):     A metal spreader or locking device was not provided on each stepladder to hold the front and back sections in an open position when the ladder was being used:      Worksite located at Sandstone and 214th Streets, Gretna NE:  A 6 foot-self supporting ladder did not have the metal spreader or locking device engaged.  The ladder was leaned against the wall so that employees could access the upper roof surface.
