OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: (a) Inside warehouse: On or about November 20, 2015 and at times prior, employees were exposed to struck by hazards while working in and around storage racks that were not permanently labeled with designated load limits. (b) Inside warehouse: On or about November 20, 2015 and at times prior, employees were exposed to struck by hazards while working in and around storage racks that were not securely anchored to the floor. (c) Joist yard: On or about November 20, 2015 and at times prior, employees were exposed to struck by hazards while working in and around storage racks that were not permanently labeled with designated load limits. (d) Joist yard: On or about November 20, 2015 and at times prior, employees were exposed to struck by hazards while working in and around storage racks that were not securely anchored to the ground. (e) Joist yard: On or about November 20, 2015 and at times prior, employees were exposed to struck by hazards while working in and around storage racks that were not installed according to manufacture instructions. (f) Joist yard: On or about November 20, 2015 and at times prior, employees were exposed to struck by hazards while working in and around storage racks that were not correctly braced against horizontal shear force.
