29 CFR 1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways or wherever turns or passage had to be made: On or about 11/23/2015, and at times prior, in the facility at 550 Cheshire Road, Pittsfield, MA, 01201, a Rough Terrain Forklift equipped with a bucket (the Lull (FL14)), a vehicle measuring eight feet wide, was driven down an aisle 10.29 feet (123.5") wide created by concrete forms on either side up to three feet (36") high with limited pedestrian egress points . The Lull FL14 was used to collect concrete debris which was shoveled by the sweeper crew into the bucket from debris piles that were positioned along the aisle. Employee "Finishers" were working in a northerly (opposite) direction standing in the aisle performing final manufacturing steps for concrete pieces. This required the Lull, traveling in a southerly direction to have to drive past the "Finishers" with insufficient space to pass safely.
