29 CFR 1910.217(b)(7)(iii): A means of selecting Off, Inch, Single Stroke, and Continuous (when the continuous function is furnished) was not by a means capable of supervision by the employer.  Production Building, Crescent St, Athol, MA  On 11/18/15 and at times prior, employees operated mechanical power presses with their keys left in the lock mechanisms for selecting Off, Inch, Single Stroke, and Continuous modes.  Employees had the ability to change modes without supervisor control.

Production Building, Crescent St, Athol, MA  On 11/18/15 and at times prior, employees operated mechanical power presses with their keys left in the lock mechanisms for selecting hand or foot modes.  Employees had the ability to change modes without supervisor control.

On 11/18/15, an employee operating a mechanical power press received a partial finger amputation injury while reaching under a light curtain into an unguarded point of operation.

Production Building, Crescent St, Athol, MA    On 11/18/15 and at times prior, employees operated mechanical power presses where inadequately configured/controlled light curtains and two hand controls did not prevent employees from contacting the point of operation.  On 11/18/15, an employee operating a mechanical power press received a partial finger amputation injury while reaching under a light curtain to hold the product being pressed.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedures and the requirements of this standard were being followed:    Crescent St, Athol, MA    As of 11/25/15, the employer had not performed the Periodic Inspection for more than two years.  The annual in-process inspection of each energy control procedure and the evaluation/refresher of each Authorized employee had not occurred.
