29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: (a)A&R Roofing, Inc. @ 1819 Harmony Heights Lane, Rapid City, SD 57702: On or about November 24th, 2015, the exposing employer did not ensure that roofing employees were using safety glasses when using pneumatic nail guns to install shingles. The condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.300(b)(2): Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating or moving parts of equipment shall be guarded if such parts are exposed to contact by employees or otherwise create a hazard. Guarding shall meet the requirements as set forth in American National Standards Institute, B15.1-1953 (R1958), Safety Code for Mechanical Power-Transmission Apparatus. (a)A&R Roofing, Inc. @ 1819 Harmony Heights Lane, Rapid City, SD 57702: On or about November 24th, 2015, the employer did not ensure that the air compressor's belt and pulley assembly were guarded. The employees were using the compressor to pressure the pneumatic air nail guns to attach asphalt shingles. The compressor was in operation at the time of the inspection with a hose connected. The condition exposed employees to lacerations and amputation hazards.

The condition exposed an employee to a fall hazard of approximately 30 feet.
