The employer did not maintain warehouse floors that had holes large enough to step into and large enough for forklift tires to become lodged when employees operated the forklifts on the loading docks. Slater Drive, Elizabeth, NJ (warehouse lower level)    Employees operated powered industrial trucks, which included a Nissan propane powered forklift which was found with defects that included, but were not limited to broken or missing propane bottle restraint straps and was not taken out of service until restored to a safe operating condition.
