29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, were not guarded. On or about November 17, 2015 at the chicken log blender, a sanitation employee was injured when he slipped while pushing a cart and put his left hand into an open hatch on the side of the blender. The rotating blades of the blender pulled the employee into the machine resulting in a fracture to his lower left arm.
