29 CFR 1926.451(c)(1):  Supported scaffolds with a height to base width ratio of more than four to one (4:1) shall be restrained from tipping by guying, tying, bracing, or equivalent means:       a)   The yellow baker scaffold was subject to tipping over when employees are on it due to the scaffold height being more than 4 times its width;  stability outriggers were not provided.  The scaffold was 29 inches wide and 139 inches tall (11 ft); the 29 inch wide scaff is required to be no more than 116 inches tall unless stability outriggers are added to increase the base width.         Location: 653 5th Ave, New York, N.Y.       On or about 10/27/2015       NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
