29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) 10 Keyland Ct., Bohemia NY: Chunker machine was not provided with a guard to prevent employees from reaching inside the machine during the operating cycle; on or about 11/28/15. Note: Because abatement of this violation is already documented in the case file, the employer needs not submit certification or documentation of this violation as normally required by 29 CFR 1903.19. Always Bagels, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR 1910.212(a)(3)(ii) on 12/18/2014 which was contained in OSHA inspection number 999480, citation number 1, item number 3 and was affirmed as a final order on 1/21/15, with respect to a workplace located at 3010 Hanford Drive, Lebanon, PA 17046.
