29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about November 25, 2015:  a) Employees installing roofing trusses were exposed to struck by hazards when the employer failed to ensure that employees receiving trusses from the crane were using proper head protection.    No abatement certification or documentation is required for this item.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  On or about November 25, 2015:  a) Employees installing roof truss materials were exposed to struck by hazards, when the employer failed to ensure that employees were using eye protection while using pneumatic nail guns to install trusses.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about November 25, 2105:  a) Employees were exposed to fall hazards of approximately 8.2 feet while installing roof trusses on a new residential structure, when the employer failed to ensure that employees were using fall protection measures.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
