29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about May 18, 2017:  a) Employer failed to properly train all employees that were exposed to fall hazards to recognize and minimize the hazard according to the standards. Employees were exposed to fall hazards of approximately 10.7 feet while measuring and laying out the locations of the second floor walls.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about May 18, 2017:  a) Employees were exposed to fall hazards of approximately 10.7 feet when the ladder they were utilizing to gain access to the upper working area did not have the rails extending above the upper landing the prescribed three feet.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  On or about May 18, 2017:  a) Employees were exposed to fall hazards of approximately 10.7 feet while measuring and laying out second floor walls on a new residential structure. The employer failed to ensure that employees were properly protected with fall protection means.  Reyes Framing, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1109100, citation number 1, item number 3 and was affirmed as a final order on April 11, 2016, with respect to a workplace located at 4773 Lost Creek Dr. in Gainesville, GA
