29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    (a) Fill room mixing area - On or about 11/19/15, a mixer employee's head, torso, arms, and legs were exposed to liquid splashes from chemicals such as, but not limited to, 93% sulfuric acid (pH of 0.3) and 45% potassium hydroxide (pH of 12 - 14) when transferring them from a bucket to a stainless steel mixing tank.  The employer did not ensure that the employee wore appropriate chemical resistant hooded coveralls and appropriate chemical resistant boots.      ABATEMENT DOCUMENTATION REQUIRED
