29 CFR 1910.95(i)(5): The employer did not ensure proper initial fitting and supervise the correct use of all hearing protectors: a) Wright Pallet, Inc. - Lima, Ohio: On or about December 2, 2015, the employer failed to ensure that employees tearing down pallets with horizontal band saws were wearing hearing protectors correctly. Employees operating the band saws were exposed to occupational noise at an 8-hour time weighted average of 98.2 dBA, which exceeded the OSHA permissible exposure limit of 90.0 dBA.

29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k): a) Wright Pallet, Inc. - Lima, Ohio: On or about December 2, 2015, the employer failed to ensure that employees exposed to occupational noise received training on the effects of noise on hearing; the purpose of hearing protectors, the advantages, disadvantages, and attenuation of various types, and instructions on selection, fitting, use, and care; and the purpose of audiometric testing, and an explanation of the test procedures. b) Wright Pallet, Inc. - Lima, Ohio: On or about December 2, 2015, the employer failed to provide training on hearing conservation to employees exposed to occupational noise in a language they can understand.
