Russell Standard Asphalt Plant, 3580 Wheelertown Rd., Waterford, PA: On or about August 24, 2015, an employee hospitalization was not reported to OSHA within 24 hours. The accident and resulting hospitalization took place on or about August 24, 2015. The incident was not reported to OSHA until November 20, 2015.
