OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazard of contact with fire: (a) Location - 150 Sohier Road, Beverly, MA, Bldg. 5A: On or about 12/2/2015, Class C ovens using a flammable special atmosphere were not designed to minimize the unintended infiltration of air. (b) Location - 150 Sohier Road, Beverly, MA, Bldg. 5A: On or about 12/2/2015, the processes and equipment for controlling flows of flammable special atmospheres to a Class C oven were not designed and operated to maintain positive pressure. (c) Location - 150 Sohier Road, Beverly, MA, Bldg. 5A: On or 12/2/2015, venting of unwanted flammable special atmosphere gas was not accomplished through controlled venting to an approved location outside of the building, or by burning the atmosphere gas and venting the products of combustion to an approved location.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) Location - 150 Sohier Road, Beverly, MA, Bldg. 5A: On or about 12/2/2015, effective shields and barriers were not provided to protect employees from contact with fire in the event of the ignition of the flammable special purpose atmosphere present during induction brazing processes.
