29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) A trench was found at 8529 N Highland, Kansas City, Missouri, and a crew was replacing an existing main waterline. A ladder was not provided for egress. Abatement verification has been provided by the employer on 12/3/2015.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that that: (a) A trench located at 8529 N Highland, Kansas City, Missouri, at an employee was observed working in a trench and was not protected from cave-ins by an adequate protective system, or benching. The trench was 9' long, 6' wide and 6' deep. There was not any cave-in protection provided. Abatement verification was provided by the employer on 12/03/2015.
