29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On December 3, 2015 employees were not protected from a fall while performing roofing operations on a steep pitch commercial roof, exposing employees to a fall of 12.1 feet.   Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about December 3, 2015 employees engaged in roofing operations were unable to recognize or minimize the hazards associated with a fall, exposing employees to a fall.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
