29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points:       a.)         Haysite Reinforced Plastics, LLC., 5599 New Perry Hwy, Erie, Pa 16509-West Wall of mixing room/department(Emergency Exit #22)-On or about December 3rd, 2015 and times prior thereto: The width of a path of egress between items including, but not limited to, production equipment(Mixer) and a pallet was approximately 24.5 inches.  The path of egress led to an emergency exit door (#22) on the west side of the mixing room.

29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces:       a.) Haysite Reinforced Plastics, LLC., 5599 New Perry Hwy, Erie, Pa 16509-Production areas-On or about December 3rd, 2015 and times prior thereto: Employees periodically entered into permit required confined spaces including, but not limited to, a Pit area in the mixing department and the Dantherm Filtration NFP dust collection unit.  The employer did not identify the pit area in the mixing department and the Dantherm Filtration NFP dust collection unit as permit required confined spaces for employees that performed work operations including, but not limited to, maintenance.
