29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form.   On or about December 1, 2015: Employees working at the facility located at 2701 N. Decatur Road, became ill after exposure to floor stripping and cleaning chemicals such as but not limited to Spartan Step Down (floor stripper with benzyl alcohol), Shineline Emulsifier Plus, The Fixx (acrylic polymer), and Sunswept (styrene/acrylic copolymer).  The employer did not adequately access the respiratory hazards within the workplace.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   On or about December 1, 2015:  Employees working in Environmental Services are exposed to corrosive chemicals such as, but not limited to Spartan Step Down stripping solution. The employer did not have an adequate means of flushing of the eyes in an emergency.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.    On or about December 1, 2015: Employees working in Environmental Services are exposed to smoke inhalation and fire hazard as well as trip hazards where the fire extinguisher was:  (a) On the wall near the thermostat but was not identified by signage. (b) Blocked by a floor scrubbing machine, a broom, and wet-floor signs, subjecting the employees to injury.
