29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a)  At Lot 15 The Cottage at Romar, Orange Beach, Alabama:  On or about December 1, 2015 and at times prior; the employer exposed his employee to a fall hazard in that he allowed the employee to tie off to an elevated aerial lift basket leaving 16 foot of rope between the tie off point and the employee thus allowing the employee to fall more than 6 feet.  The fall hazard was in excess of 40 feet to the ground below.
