29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about August 2, 2016; residential construction at 314 #3 E 1st Ave, Gulf Shores, Al: The employer failed to ensure employees involved in securing trusses on a new residential home used fall protection which exposed employees to the hazards associated with a fall of 32' from a 6:12 pitch roof to the ground.  b) On or about August 2, 2016; residential construction at 314 #2 and #3 E 1st Ave, Gulf Shores, Al: The employer failed to ensure employees involved in installing sheathing on a new residential home used fall protection which exposed employees to the hazards associated with a fall of 32' from a 6:12 pitch roof to the ground.  c) On or about August 2, 2016; residential construction at 314 #2 and #3 E 1st Ave, Gulf Shores, Al: The employer failed to ensure an employee was protected from fall hazards of 23'1" when he walked across a plank connecting the two buildings at the second floor.  d)  On or about August 2, 2016; residential construction at 314 #2 and #3 E 1st Ave, Gulf Shores, Al: The employer failed to ensure an employee conducting work from the second level of both buildings was protected from falling 21'1/8" due to missing guard rails on building #3 and no guardrails on building #2.

29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.  a) On or about August 2, 2016 at 314 E 1st Ave, building #2, Gulf Shores, Al: The employer did not ensure an employee using personal fall arrest were properly anchored in that their rope was wrapped around a support beam and the snap hook was connected back to the rope while taking measurements and installing sheathing which exposed him to falls of 32 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about August 2, 2016 at 314 E 1st Ave, building #2 and #3, Gulf Shores, Al: The employer failed to ensure the job made ladders used to access both buildings 1st floor extended 3 feet above the landing area exposing employees to falls of 10 feet.
