29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)  At the jobsite, on the roof, on or about 12/4/15:  Workers were performing roofing work on a 5:12 pitch roof with an approximate ground to eave height of 20 feet without fall protection of any kind.   b)  At the jobsite, on a porch/entrance roof, on or about 12/4/15:  A worker was performing roofing work on a 5:12 pitch roof with an approximate ground to eave height of 10 feet without fall protection of any kind.   c)  At the jobsite, on the roof, on or about 12/8/15:  Workers were performing roofing work on a 5:12 pitch roof with an approximate ground to eave height of 20 feet without fall protection of any kind.     Abatement certification  must be submitted for this item.

Abatement certification  must be submitted for this item.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  a)  At the jobsite, on the roof, on or about 12/8/15:  Employees were performing roofing work on a 5:12 pitch roof with an approximate ground to eave height of 20 feet where a second lifeline was connected to a single snaphook that was connected to one anchor device and where that anchor device was not connected appropriately to the roof ridge.  This use would not ensure 5,000lbs per person, and was not attached as the design would dictate.    Abatement certification  must be submitted for this item.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a)  At the jobsite, on the roof, on or about 12/8/15:  Employees were performing roofing work on a 5:12 pitch roof with an approximate ground to eave height of 20 feet where lifelines and rope grab devices were used with excessive slack in the lines, and where that slack would have ensured a greater than 6 foot free-fall and likely contact with the ground.    Abatement certification  must be submitted for this item.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  At the jobsite, on a porch roof, on or about 12/4/15:  A worker accessed a roof with an approximate 10 foot ground to eave height with an extension ladder where that ladder did not extend at least three feet above the upper landing.
