Buildings 3426 and 3446; on or about 01 December 2015 and times prior; the employer did not ensure that emergency exits lights were in proper working order. Four instances were noted.        a. Building 3426, wood shop, three instances were noted; situated above the exit sign, and at stations labeled nine and twelve; the emergency exit lights were inoperative.       b. Building 3446, intake area, the emergency exit lights were inoperative.  This condition exposed employees to delays in exiting the building causing smoke inhalation and fire hazards.

29 CFR 1960.8(a): 29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm.  Buildings 3426 and 3446; on or about 01 December 2015 and times prior thereto; warehouse pallet storage racks were not secured to the wall or floor. Two instances were noted:        a. Building 3426, Supply Warehouse, inbound and outbound area, ten warehouse pallet racks; make and model unknown were not secured to the floor or wall. Some had visible damage to the cross beams. Powered industrial vehicles are use in this area.       b. Building 3446 Hazmat Pharmacy, adjacent to room 14B, two Husky brand Model IBX43096 warehouse pallet racks were not secured to the wall or floor. The diagonal struts and cross beams had visible damaged. Powered industrial vehicles are utilized in this area.  This condition exposed employees to a struck by hazard.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.  Building 3446, Intake area; on or about 01 December 2015 and times prior thereto; A knockout plug was missing from the left hand side of a energized electrical outlet box. This condition exposed employees to electrical shock and electrical burn hazard.  The Air Force was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.305(b)(1)(ii), which was contained in OSHA inspection number 111281, citation number 1, item number 4 and was affirmed as a final order on 06 March 2012, with respect to a workplace located at Eielson Air Force Base, 354th Civil Engineering Squadron, Structures shop panel C; Utilities shop entryway, and Electrical shop.
