29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter:  In the field and in detention areas, agents, who were required to wear respirators when handling suspects with suspected tuberculosis, were not fit tested at least annually, exposing employees to the hazard of contacting tuberculosis.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.:  Near the west wall of the vehicle maintenance shop, there was no guard for the rotating chuck on a Wilton 15 inch Variable Speed, floor standing drill press.  Employees, who used the press to grind metal parts for holding equipment in vehicles cabs, were exposed to the hazard of contacting the rotating chuck.
