29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards:     a)  On or about November 17, 2015, the employer did not assure employees working on the storage area above the break room were protected from fall hazards.  Employees were exposed to a 10 foot fall to the shop floor.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     a)  On or about November 17, 2015, the employer did not have a lock out program for the control of the hazardous energy, which includes machine specific procedures, training, and periodic inspections, while installing, inspecting and servicing spreaders in new and existing grain bins.  Failure to lock out the energy sources while these tasks are being performed exposes employees to caught-in hazards.  An example of this violation is when employees performed work at a client's site in Congerville, Illinois.

29 CFR 1910.22(d)(1): On every building or other structure, or part thereof, used for mercantile, business, industrial or storage purposes, the loads approved by the building official were not marked on plates of approved design and securely affixed in a conspicuous place in each space to which they relate.  a)  On or about December 8, 2015, the employer did not assure that the two raised storage areas had a label for the capacity limit.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a)  On or about December 15, 2015, the employer did not assure that employees who operate the forklifts completed a training program and received a performance evaluation.  The employees use the lifts to carry products throughout the facility and to load and unload trucks.
