29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces:      1.) On or about November 17, 2015,  the employer did not ensure that the workplaces were evaluated to determine if any structures or equipment that employees enter while performing their job were confined spaces and if any of the structures or equipment were permit-entry confined spaces.  An example of this violation is for a confined space entry at a client site located in Congerville, Illinois.

29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146:       1.)  On or about November 17, 2015, the employer did not develop and implement a confined space entry program.  Employees regularly enter customers grain storage bins to install or repair equipment.  The program should include, but not be limited to, evaluation of the hazards associated with the permit space, provide the necessary equipment to safely enter the space, provide training, develop and implement procedures for summoning rescue and emergency services, and annually review the program and implement any improvements.  An example of this violation is for a confined space entry at a client site located in Congerville, Illinois.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields:      1.)  On or about December 8, 2015, the employer did not ensure employees working near the weld department were protected from stray ultraviolet radiation from the arc rays by not providing welding curtains.  Employees assembling parts near the welding area were not protected from arc rays during the welding process.
