29 CFR 1917.112(b)(3): Where guardrails were impracticable due to machinery requirements or work processes, an alternate means of protecting employees from falling, such as nets, was not used: a) Berth #1, Shore to Ship Crane #1 - Employees were standing on top of a trolley beam located on the top back reach of a Somitomo shore to ship crane while replacing a clutch coil and not protected from a 132 ft., fall hazard, on or about 12/07/2015. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1917.151(b)(7): The power supply to machines was not turned off, locked out and tagged out during repair, adjustment, or servicing: a) Birth #1 - An employee was replacing a clutch coil on the top back reach on a Somitomo shore to ship crane and the power supply that operates the assembly was not locked and tagged out, on or about 12/07/2015. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification.
