29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) The Bellevue Manufacturing Company located at 300 Ashford Ave. Bellevue, OH 44811; On December 01, 2015, the employer did not ensure employees were protected while performing setup and maintenance activities on hydraulic Straight Draw Press #4-7. Employees were required to perform setup activities on the hydraulic press and relied on a keyed switch, exposing them to unexpected start up of the equipment.
