Employees using the underhung crane without the benefit of known lifting capabilities were exposed to struck by and crushing hazards while lifting and moving dies in Tool Room.  b. The Bellevue Manufacturing Co. on or about September 1, 2022 at the address of 300 Ashford Avenue, Bellevue, OH: The monorail of the CM underhung crane S/N L-2T03SD was not evaluated and rated to determine the maximum load capabilities. Employees using the underhung crane without the benefit of known lifting capabilities were exposed to struck by and crushing hazards while lifting and moving dies in Tool Room.
