OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing hazards while operating a powered overhead crane:  a. The Bellevue Manufacturing Co. on or about September 1, 2022 at the address of 300 Ashford Avenue, Bellevue, OH: The monorail of the Coffing underhung crane S/N: EC1H45759UL was not evaluated and rated to determine the maximum load capabilities. Employees using the underhung crane without the benefit of known lifting capabilities were exposed to struck by and crushing hazards while lifting and moving dies in Tool Room.  b. The Bellevue Manufacturing Co. on or about September 1, 2022 at the address of 300 Ashford Avenue, Bellevue, OH: The monorail of the CM underhung crane S/N L-2T03SD was not evaluated and rated to determine the maximum load capabilities. Employees using the underhung crane without the benefit of known lifting capabilities were exposed to struck by and crushing hazards while lifting and moving dies in Tool Room.

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:   a. The Bellevue Manufacturing Company, 300 Ashford Avenue, Bellevue, Ohio 44811: On or about September 1, 2022,  the employer did not ensure periodic audits of the energy control procedure were completed at least annually.  Employees are required to perform servicing and maintenance activities on machines including but not limited to:  Aida 1 & 2 blankers, Reverse draw presses, Straight Draw Presses  and Farr/Transmat washers which require energy control procedures that have not been reviewed for deficiencies there by exposing employees amputation hazards.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. The Bellevue Manufacturing Company, 300 Ashford Avenue, Bellevue, Ohio 44811: On or about September 1, 2022, the employer failed to ensure the AIDA 1 Blanker conveyor was guarded to protect employees from a pinch point.  Employees stacking blanks off the conveyor were exposed to a pinch point at the one inch gap between the conveyor's end and the chute.

29 CFR  1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided:   a. The Bellevue Manufacturing Company, 300 Ashford Avenue, Bellevue, Ohio 44811: On or about September 1, 2022, in the Tool Room, employees were exposed to electrical shock while utilizing a locally manufactured temporary wiring system. Employees using an extension cord, fitted with a steel 4" square electrical box, designed for permanent installations was being used to power the "Rockonizer."  The electrical enclosure used, was not designed for this purpose.
