29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  Wood Working Facility: On or about December 9, 2015, employee(s) who perform wood lamination processes were exposed to chemical hazards from the use of Denatured Alcohol (Ethyl Alcohol 30-50%, Methanol 40-60%), and Standard Lacquer Thinner C160 36 (toluene, isobutyl acetate, Methyl Isobutyl Ketone, 2-Methyl-1-proponal, 2-Butoxyethanol), which contain skin irritants and permeators, without the use of personal protective equipment, such as chemical resistant gloves.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    Wood Working Facility: On or about December 9,2015, and times prior thereto, employees who were exposed to hazards from the use of chemicals such as, but not limited to Denatured Alcohol (Ethyl Alcohol 30-50%, Methanol 40-60%), Standard Lacquer Thinner C160 36 (toluene, isobutyl acetate, Methyl Isobutyl Ketone, 2-Methyl-1-proponal, 2-Butoxyethanol), and Tensor Grip L10N Non-Flam High Temp Contact Adhesive (Methylene Chloride 60-100%), were not informed by the company of the associated hazards of these chemicals, symptoms of exposure, and precautionary measures.
