Condition exposed an employee to an amputation hazard.

(a) Western Sugar Cooperative, at 18317 Hwy 144, Fort Morgan, CO 80701; On and before December 9, 2015, the employer failed to ensure that authorized employees received training in the recognition of applicable hazardous energy sources and the methods and means for energy isolation and control. Specifically, the employer failed to train his employees in the recognition of the potential for stored energy of the skip pit bucket rock slide door, resulting in an employee placing his hand in a hazardous location beneath the door and injuring himself.

29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee failed to verify that isolation and deenergization of the machine or equipment had been accomplished. (a) Western Sugar Cooperative, at 18317 Hwy 144, Fort Morgan, CO 80701: On and before December 9, 2015, the exposing employer failed to ensure that prior to starting work on machines or equipment that had been locked out or tagged out, isolation and deenergization of the machine or equipment had been accomplished. Condition resulted in an employee injuring himself when he placed his hand in the slide door of a skip bucket which had residual energy of position which had not been neutralized, the door released its energy and injured the employee.
