Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to the hazards of being struck by fragments of Polyvinyl Chloride (PVC) plastic pipe in the event that the PVC plastic pipe ruptured:      a)      Polar Service Center:  On or about December 8th, 2015 and at times prior to, the employer exposed employees to the hazards of being struck by fragments of Polyvinyl Chloride (PVC) plastic 1 inch pipe in the event that the PVC plastic pipe ruptured.  Plastic pipe was used in above ground locations in the facility to transport compressed air from compressor through various diameter PVC lines at 160 psi.      The employer failed to follow ASME Gas Transmission and Distribution Piping System ASME B31.8-2010, Section 842.3.3 which prohibits the above ground use of PVC pipe for compressed air conveyance.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that employees were exposed to the explosion and asphyxiation hazards from employees required to use a Gas Clip multigas detector before each use entering confined spaces of chemical and fuel tanker trucks without being afforded calibration gas to bump test the instrument to assure the detector was operating as manufacturer designed and the calibrator was past the calibration date last done by contracted service Safety Source.      a)      Polar Service Centers:  On or about December 8th, 2015 and at times prior to, the employer exposed employees to the explosion and asphyxiation hazards while entering chemical and fuel tanker trucks by using a Gas Clip multigas detector Model MGC-P, SN: M4PF060192 that was not bump tested before each use and was past calibration date last performed by contracted service to be due on October 2nd, 2015.

PSC Custom LP dba Polar Service Centers, was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1910.132(d)(1), which was contained in the OSHA inspection number 994444, Citation 2, item 1 and was affirmed on final order date January 21st, 2015, with respect to a workplace located at 86 Telegraph Road, Prichard, AL 36610.
