29 CFR 1910.178(n)(1): All traffic regulations shall be observed, including authorized plant speed limits. A safe distance shall be maintained approximately three truck lengths from the truck ahead, and the truck shall be kept under control at all times. Warehouse, powered industrial vehicles: on or about December 5, 2015, and on dates thereafter, forklift traffic patterns were not regulated to maintain safe distances between moving forklifts. Onboard computers, with functionality during travel, created distractions that affected operators ability to keep their vehicles under control at all times. Employees were exposed to struck-by hazards likely to result in multiple traumatic injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(n)(6): The driver shall be required to look in the direction of, and keep a clear view of the path of travel. Warehouse, powered industrial trucks: on or about December 5, 2015, and on dates thereafter, employees were exposed to struck-by hazards when incoming message sounds and allowed functionality of the computers during travel distracted forklift operators from looking in the direction of travel. Such distractions affect safe operation and have resulted in collision and multiple traumatic injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
