29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301 or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: (a) The employer did not ensure that each recordable injury or illness was entered on the OSHA 300 Log within seven (7) calendar days of receiving information that a recordable injury had occurred for the 2015 calendar year. The information had been saved on a spreadsheet and the recordable injuries and/or illnesses had not been transferred to the 2015 OSHA Log. The Injury Tracking Log did not contain all the information that is listed on the OSHA 300 Log. This violation was abated during inspection. Abatement certification for this violation is not required.
