29 CFR 1926.1053(b)(4): Ladder was not used only for the purpose for which it was designed: a) Hampton VA Medical Center, 100 Emancipation Drive Hampton, VA, 23667 at Building 137A Penthouse Mechanical Room: On or about 12-09-15 and times prior thereto, a six foot fiberglass "A" Frame style ladder was in use in the closed position and leaning up against piping while an employee climbed one to two rungs high to obtain measurements in the overhead piping mechanical area. The ladder was footed on an elevated concrete pad with the ladder foot less than one inch away from the concrete pad edge. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
