29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:    a) Block 1, Lot 34: On December 9, 2015 and at times prior thereto employees were exposed to fall hazards while engaged in framing activities on a 6 in 12 pitched roof without a fall protection system in place.  Fall hazards ranged from 10 feet to over 25 feet.      b) Block 1, Lot 35: On December 9, 2015 and at times prior thereto employees were exposed to fall hazards while engaged in framing activities on a 6 in 12 pitched roof without a fall protection system in place.  Fall hazards ranged from 10 feet to over 25 feet.        c) Block 1, Lot 35: On December 9, 2015 and at times prior thereto a fall protection system was not in place on the second floor stairway landing.  This exposed employees engaged in framing activities on the second floor, to a 10 foot fall hazard.     Note) Certification of corrective actions ARE required for this item.

29 CFR 1926.503(a)(2)(iii) The employer did not assure that each employee has been trained, as necessary, by a competent person qualified in the use and operation of guardrail systems or personal fall arrest systems:  a) Residential jobsite, Block 1 Lot 34 & 35: On December 9, 2015 and times prior thereto employees did not recognize or acknowledge the hazards of performing framing activities over 6 feet without the use of a fall protection system.  Employees did not take necessary actions to minimize or correct the hazards.     Note) Certification of corrective action IS required for this item.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:    a) Block 1, Lot 34, front of house: On December 9, 2015 an extension ladder used for access to the roof did not extend three feet above the eave of the roof.       b) Block 1, Lot 35, front of house: On December 9, 2015 an extension ladder used for access to the roof did not extend three feet above the eave of the roof.
