Avenel, NJ site - Warehouse:    Employer failed to ensure that employees who pick orders by climbing through the storage rack shelves, are provided with personal protective equipment such as but not limited to a safety harness and lanyard or other fall arrest system.     Condition noted on or about 1/23/14.    STRETCH-O-RAMA WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD, 29 CFR 1910.132(a), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 955884, CITATION NUMBER 1, ITEM NUMBER 3, ISSUED ON 07/01/2014, AND WAS AFFIRMED AS A FINAL ORDER ON 8/1/2014, WITH RESPECT TO THE SAME WORKPLACE LOCATED AT 5 PADDOCK STREET, AVENEL, NJ 07001.
