29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300.    a)On or about 12/9/2015 James Marine (dba Walker Boat Yard) at 4040 Clarks River Road in Paducah, KY had not entered complete information on the 2015 OSHA 300 LOG Column F as to the object/substance that directly injured or made person ill:     1. OSHA 300 Log 2015 Column (f): cases  5 (dated 8/3/15); 6 (dated 8/13/15); 7 (dated 9/24/15); 8 (dated 10/26/15); and 9 (dated 11/2/15).     In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1200(e)(1)(i): The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet.  a) On or about 12/10/2015 and at times prior to James Marine (dba Walker Boat Yard) employees working at 4040 Clarks River Rd in Paducha, KY were exposed to various hazardous materials (listed as eye, skin, and respiratory irritants) and the employer did not compile a complete list of the hazardous chemicals known to be present  .  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
