29 CFR 1910.132(a): Protective equipment was not maintained in a sanitary and reliable condition: Drilling rig derrick: On or about November 27, 2015, the ladder safety sleeve portion of the derrick ladder fall protection system used by employees climbing the derrick ladder was heavily soiled and exhibited signs of notable wear. This exposed the employees to the hazard of falling while using the derrick ladder.
